: This document lists schools that were recognized by Level of Recognition (Green, Model, Exemplar) and by county! Consider this list when referring Coordinators/Coaches to implementing schools - I would recommend schools at Model level or above for initial visits!
Partnership Agreement
A Partnership Agreement is required to be on file for every trained PBIS school. The agreement is a 3-5 year commitment from the school administrator and LEA Superintendent to implement PBIS with fidelity and to submit annual ODR data to NC DPI.

Self-Assessment Survey (or EBS Survey)
The EBS Survey is used by school staff for initial and annual assessment of effective behavior support systems in their school. The survey examines the status and need for improvement of four behavior support systems: (a) school-wide discipline systems, (b) non-classroom management systems (e.g., cafeteria, hallway, playground), (c) classroom management systems, and (d) systems for individual students engaging in chronic problem behaviors
- This is a 'cheatsheet' for school faculty to access the survey with their provided school ID, and some simple directions to complete the survey.

Implementation Inventory Online
The Implementation Inventory Online is completed at least annually - once by November 30th, and then prior to the Schoolwide Evaluation. This tool assesses implementation at the universal, secondary and tertiary levels. Level of implementation is determined by score earned at each stage of training and implementation (i.e., Level 2: Universal - 80%+, Secondary - 80%+, Tertiary - <80%).
IIO should be submitted using the Data Management System.

Schoolwide Evaluation Tool
The Schoolwide Evaluation Tool (SET) may be used pre-implementation to establish a baseline of implementation, and/or for post-assessment. Schools strive to earn 80% or better on the SET by means of interviews (Administrator, staff, and students), data review, and direct observation.
SET scores should be entered into the Data Management System. You may want to also enter into www.pbisassessment.org for comparison data and graphs.
